Conflict Free Service Coordination

ISN’s Service Coordination Team provides coordination and oversight of services for individuals over the age of 21 with Intellectual & Developmental Disabilities or an Acquired Brain Disorder. ISN works collaboratively and in close partnership with local Area Agencies to offer Service Coordination to eligible and interested individuals.

At the core level Service Coordination requires that assessment and coordination of services are separate from the delivery of services, with the goal to limit any conscious or unconscious bias a care manager or agency may have, and ultimately promote the individual’s choice and independence.

At this time ISN is offering Service Coordination services to the greater areas of the following counties: Sullivan, Hillsborough, Merrimack and Rockingham. However, we welcome referrals from anywhere in New Hampshire.
